CCPA / CPRA Notice & Opt-Out

Effective: June 1, 2026

This section applies to California residents and describes rights under the California Consumer Privacy Act, as amended by the CPRA.

Medical information governed by HIPAA and California's Confidentiality of Medical Information Act (CMIA) is exempt from the CCPA. This notice therefore covers your non-exempt data — primarily website-visitor, prospective-patient, and marketing/analytics data — not the protected health information handled inside the care relationship, which is covered by our HIPAA Notice of Privacy Practices.

Categories of personal information we collect

For non-exempt data: identifiers (e.g., name, email, IP address), internet/device activity, commercial information, and inferences drawn from the above. Sources, purposes, and recipients are described in our Privacy Policy.

Your California rights

  • Right to know what personal information we collect, use, and disclose.
  • Right to delete personal information we hold about you, subject to exceptions.
  • Right to correct inaccurate personal information.
  • Right to opt out of "sale" or "sharing" of personal information for cross-context behavioral advertising.
  • Right to limit use of sensitive personal information.
  • Right to non-discrimination for exercising these rights.

How to submit a request

[email protected]. We will verify your identity before responding. You may use an authorized agent.
